A Smithtown woman filled out a prescription and forged a doctor's signature at a local drug store, according to police.
Suffolk County police said Yvonne Munoz 38, of Smithtown, was charged with second degree forgery when she presented a fake prescription and singed it with a doctor’s name at the CVS pharmacy in Kings Park last Wednesday.
Police also charged Munoz with third degree grand larceny for an unrelated incident. Police said Munoz took cash payments from an office on Terry Road in Smithtown and replaced them with fake credit card receipts between June of 2009 and May of 2012.
Munoz was arraigned on Sept. 4 at Suffolk First District Court and released on $30,000 bond.
